This concept is usually discussed as clinical equipoise, meaning the uncertainty exists across the relevant expert community rather than in the mind of a single physician. It is fundamentally an ethical idea about what is and is not yet known.
Why does Equipoise matter in clinical research?
Equipoise provides the ethical justification for randomizing patients in a clinical trial. If the community genuinely does not know which arm is better, then assigning patients to one treatment or another does not knowingly deny anyone a superior option.
When equipoise no longer holds, for example once interim results clearly favor one arm, continuing to randomize patients becomes ethically problematic. Monitoring whether equipoise still exists is therefore part of conducting a trial responsibly.
