Accelerated Approval

An FDA pathway allowing earlier marketing of drugs for serious conditions based on a surrogate endpoint reasonably likely to predict clinical benefit. Manufacturers must complete confirmatory trials to verify the expected real-world outcome.

What is Accelerated Approval?

Accelerated Approval is a regulatory pathway used by the U.S. Food and Drug Administration to allow earlier marketing of drugs that treat serious or life-threatening conditions and address an unmet medical need. Rather than waiting for direct evidence of clinical benefit, the FDA can grant approval based on a surrogate endpoint that is reasonably likely to predict that benefit.

A surrogate endpoint is a marker, such as a laboratory measurement or imaging finding, that stands in for an outcome patients care about, like survival or symptom relief. Approval on this basis comes with a commitment that the manufacturer will conduct confirmatory studies to verify the real-world benefit.

Why does Accelerated Approval matter?

The pathway exists to shorten the wait for promising therapies in areas where patients have few or no good options, such as certain cancers and rare diseases. By relying on surrogate measures, it can bring treatments to patients years earlier than a conventional approval timeline would allow.

The tradeoff is that early evidence may not always translate into the expected clinical benefit, which is why confirmatory trials and the possibility of withdrawal are integral to the framework. This balance between speed and certainty makes Accelerated Approval a recurring focus of discussion among regulators, manufacturers, and payers weighing coverage.
