Outmigration

The movement of patients out of a local market or health system to receive care elsewhere, often for specialty or surgical services unavailable nearby. Health systems track outmigration to identify lost volume and revenue, and to guide decisions about adding service lines or facilities.

What does outmigration mean?

Outmigration refers to patients traveling outside their local market or health system to obtain care elsewhere. It frequently happens for specialty or surgical services that are not available, or not perceived as strong, close to home.

The term describes a movement of volume away from local providers, whether because of capability gaps, reputation, referral patterns, or patient preference. It is most often discussed at the level of a market, region, or individual health system.

Why do health systems track outmigration?

Outmigration represents lost patient volume and the revenue that would have accompanied it, so systems monitor it to understand where and why patients are leaving. Identifying these patterns highlights service lines that may be underbuilt or referral relationships that are sending cases away.

These insights guide strategic decisions, such as adding a specialty, recruiting physicians, or opening a new facility to recapture the departing volume. For organizations that include or partner with ambulatory surgery centers, understanding outmigration can reveal opportunities to keep surgical cases local.
