Physician Preference Item (PPI)

A Physician Preference Item (PPI) is a high-cost surgical supply or implant, such as a joint prosthesis or stent, selected based on the operating surgeon's preference. PPIs represent major supply-cost and margin drivers for ambulatory surgery centers.

What is a Physician Preference Item (PPI)?

A Physician Preference Item (PPI) is a high-cost surgical supply or implantable device chosen primarily according to the operating surgeon's preference rather than on price or standardization alone. Joint prostheses, spinal hardware, and cardiac stents are typical examples.

Because surgeons often have strong, training-based preferences for specific products, PPIs can vary widely from one physician to the next even for the same procedure. This variation makes them difficult to standardize through ordinary purchasing controls.

Why do PPIs matter for surgery center margins?

Physician preference items frequently represent the single largest supply expense in an implant-heavy procedure, so they are a major driver of cost and profitability at an ambulatory surgery center. Small differences in device pricing can swing the margin on a case substantially.

Managing PPIs requires balancing surgeon autonomy against cost control through tactics like negotiated pricing, capitated agreements, or standardization where clinically acceptable. Accurate tracking of PPI usage and cost is essential to understanding the true economics of each case.
